Information Required on Receipt
A professional receipt should include:
- Clinic or practitioner name
- Business address
- Phone number and email address
- Invoice or receipt number
- Invoice date
- Client name
- Description of services or products
- Payment amount
- Payment method
- Practitioner designation and credentials
- Signature (if required)
Avoid vague descriptions whenever possible.
Product Information Requirements
For medical devices and support products, detailed product information should be included whenever possible.
